Tear trough filler
Medical treatment of the tear trough with hyaluronic acid so that your eyes stop looking tired when you are not
About the treatment
What does it involve?
There are dark circles that are a problem of volume and dark circles that are a problem of colour. They are not treated the same way, and confusing them is the reason many people end up with a result they did not expect.
This treatment is aimed at the hollow dark circle: the tear trough, the step that marks itself between the lower eyelid and the cheekbone. When it deepens, it creates a fixed shadow that light accentuates and that does not depend on how much you have slept. It is corrected by depositing a very small amount of low-density hyaluronic acid in a deep plane, usually with a cannula to respect the vessels of a very delicate area.
If your dark circle is brown, from pigment, filler is not going to remove it: there the work is to depigment and protect from the sun. And if what is showing is fat bags or there is excess skin on the eyelid, adding product can make the appearance worse rather than better.
That is why here we do not treat the dark circle, we treat your dark circle. At the medical assessment we identify which type you have, whether it is a mixture of several and whether filler is indicated. If it is not, we tell you and explain what does make sense.
Not all dark circles are treated the same way
Broadly speaking there are three origins and they almost always come mixed: the structural dark circle, which is a shadow caused by hollowing of the tear trough; the pigmentary one, brown in tone from excess melanin; and the vascular one, bluish, because the skin of the eyelid is so thin that the vessels show through. Filler only responds well to the first, and the examination is what decides.
What is assessed in the gaze
- The real depth of the tear trough
- The thickness and transparency of the eyelid skin
- Whether there are fat bags or only shadow
- A pigmentation and vessel component
- Projection of the cheekbone and support below
- Laxity of the lower eyelid
- A tendency to retain fluid and morning puffiness
- Medical history, allergies and medication
Common reasons for consultation
- People ask whether you slept badly when you slept well
- Your eyes look dull on video calls and in photos
- The shadow is accentuated by light from above
- One side more hollow than the other
- The concealer you use does not manage to cover the shadow
- The area has been emptying out since your mid-thirties
How the area is worked on
-
Examination of the lower eyelid
The area is looked at under different lights and expressions to tell shadow from pigment and rule out bags.
-
Medical history and decision
We review medical history, medication and allergies. This is where it is decided whether filler is indicated or not.
-
Marking the entry points
One or two points per side are marked and the path of the cannula is planned.
-
Disinfection and topical anaesthetic
The area is cleaned and anaesthetic cream is applied. The eyelid is delicate and we work without rushing.
-
Deep deposit of the product
We advance with a cannula and deposit very little, resting on the bone, without coming too close to the surface.
-
Checking and gentle pressure
It is distributed with gentle pressure, one side is compared with the other and local cold is applied.
-
Review at 3 weeks
It is assessed once the swelling has gone. If anything needs refining, it is done with a minimal amount.
What changes when the shadow is filled
- Less shadow under the eye
- A smoother transition between eyelid and cheekbone
- A brighter gaze without changing the expression
- A better appearance in photos with front lighting
- Less contrast at the end of the day
- Resorbable, reversible material
- No surgery, no stitches and no time off
- A result that can be assessed within a few days
Care before and after
Before the session
- Tell us if you take anticoagulants, antiplatelet drugs or anti-inflammatories
- Tell us if you have had thyroid or kidney problems or significant allergies
- Avoid alcohol and excess salt the day before
- Come with no creams or make-up on the eye contour
- Do not book it just before a trip or an important event
The following days
- Do not press or rub the eyelid for the first few hours
- Gentle local cold if you feel swelling, without pressing
- Sleep with your head slightly raised for the first two nights
- Avoid intense exercise, sauna and heat for 48 hours
- Go easy on salt so as not to retain fluid in the area
- Daily sun protection on the eye contour
In the lower eyelid what is to be expected is some swelling and the possibility of a small bruise, because it is a very vascularised area: it resolves within a few days. What is really assessed is the result after 2 or 3 weeks. If you feel pain that is increasing, a change in skin colour or any change in vision, get in touch with us immediately.
Frequently asked questions
Does it work for any type of dark circle?
No, and it is the most important question. It works when the dark circle is a shadow caused by hollowing of the tear trough. If the problem is pigment or vessels, the approach is a different one, and if there are bags or excess skin, filling is not the solution. The assessment decides.
Does injecting so close to the eye hurt?
Less than it seems. Anaesthetic cream is applied, in most cases we work with a cannula and the product usually contains a local anaesthetic. You feel pressure more than a prick.
How long does the result last?
In this area it is usually between 8 and 12 months, and in some cases longer, because it is an area of little movement. It depends on the product, the amount and your metabolism.
Can it end up swollen or with a bluish tone?
They are the two effects we watch for. If too much product is used, or it is placed too superficially, persistent swelling or a bluish reflection can remain. That is why a small amount is used, in a deep plane and in phases.
Can it be removed if I do not like it?
Yes. Hyaluronic acid is reabsorbable and, if needed, can be dissolved with a specific medical treatment. It is one of the reasons this material is chosen in such a visible area.
How many sessions do I need?
Usually one, with a later review. With very deep troughs we prefer to split it over two sessions a few weeks apart rather than forcing all the volume at once.
Can I go to work afterwards?
Yes. There may be some swelling or a red mark for the first few hours. If you are worried about it showing, leave a couple of days' margin before an important meeting.
Are there cases where it is not done?
Yes. It is not injected with an infection or an open wound in the area, nor over uncontrolled eye or skin conditions, and it is assessed with particular care in pregnancy, breastfeeding, autoimmune conditions and anticoagulant treatment. It is decided in the consultation, never over the phone.
